To Assign a Claim |

If you would like to assign a claim to Mills Mehr & Associates, there are several
ways to do so:

a. Online – Simply fill out our online form and the claim will immediately be submitted
to the Mills Mehr
General Mailbox, which is monitored constantly during business hours. Links

b. Email – You can download our Property or Liability Assignment Form, fill it out
and email it directly
to the Mills Mehr General Mailbox at mma@millsmehr.com

c. Fax – Again, you can download either Assignment Form and fax it to our office

d. Phone – If you would like to assign a claim via telephone or would like to speak
to someone prior
